Information you choose to provide
The contact form may collect your name, email, phone number, preferred reply method and the message you write. Please do not send medical records, test results, insurance details, government identification numbers or emergency information.
How information may be used
- To respond to inquiries and requests.

- To comply with law or protect the rights and safety of the organization and others.
Health information
This website is not a patient portal and is not designed to receive protected health information. Faith Fertility is presented as an educational and community support organization, not a healthcare provider.
Local storage and contact drafts
The contact form saves a draft in your browser and opens your email app with a message addressed to jacob@fertilityfaith.com. The website does not send the message automatically; you must send it from your email app. Clearing browser storage removes the local draft.
Third-party services
Embedded maps and externally hosted fonts may receive technical information such as an IP address, device information and referring page according to their own policies.
Security and retention
No internet system is perfectly secure. When form delivery is activated, the organization should adopt documented access, retention and deletion practices appropriate to the information collected.
Your choices
You may avoid using the form, request correction or deletion of information previously sent, and limit browser cookies or storage through your device settings.
Children
This website is intended for adults and is not knowingly designed to collect personal information from children under 13.
